Many modern women, and an increasing amount of men, have unrealistic images of themselves. The model looks at a magazine, disgusted with her imagined self image, to the extent of writing her hatred of being so 'fat'.
Viewers of the image see the reality of the situation, and feel sadness and fear where this situation may be heading. In that it invokes emotion so easily, it is a prime example of modern art.
